Supporting Families Through the Transplant Journey
Transplant social workers provide essential emotional, social, and practical support throughout the transplant journey. They assess patients’ readiness, offer emotional counseling, educate on the process, and navigate financial or resource challenges. Acting as a bridge between patients, families, and healthcare teams, they help ensure better outcomes and improved quality of life for transplant recipients.
The Caregiver Lifeline Program offers live continuing education programs for transplant and dialysis workers throughout the year to help ensure they stay current with medical advancements, mental health techniques, and cultural competency. This helps them provide accurate information, effective counseling, ethical guidance, and access to resources, ultimately improving patient care and outcomes throughout the transplant process.
We offer live continuing education programs for social workers throughout the year.
Organ transplant is a complex, lifelong process. We provide Transplant Professionals with continued education and resources to support their transplant patients.
