  • Develop effective strategies to educate patients about kidney transplantation to improve patient listings.

EQRS, SRTR, and other Network Updates – Tricia Phulchand – (1 hour)

• • Discuss and demonstrate sources available to track patient status and transplant center performance rankings
  • Understand how EQRS data can be accessed and utilized to assist dialysis social workers in supporting ESRD patients through the transplant process.
  • Review how SRTR can be utilized by dialysis SW to support patients’ transplant listing
